- 1999-07-16: About 9:41 p.m., on approach toward Martha's Vineyard over open ocean, the aircraft enters a rapid descending right turn, a 'graveyard spiral,' and dives into the Atlantic roughly 7.5 miles southwest of Gay Head, descending at times faster than 4,700 feet per minute. All three occupants are killed on impact.
