That because ethylene oxide is a carcinogen and is used to sterilise medical swabs and brushes, patients undergoing cervical screening or Covid-19 testing are being exposed to a cancer-causing chemical, and the risk is not being disclosed to them.
The short answer

Contradicted. The chemistry in this claim is real and the conclusion drawn from it is not. Ethylene oxide is a gas used worldwide to sterilise medical devices, and overexposure to it genuinely can cause cancer and lung damage, which is why occupational exposure is tightly controlled. None of that establishes that a device sterilised with it carries a risk to the patient who touches it, and the regulator says plainly that it does not. Asked about the cervical screening version in August 2026, a spokesperson for the UK's Medicines and Healthcare products Regulatory Agency told Full Fact there is no credible evidence that the swabs used in cervical smear tests are associated with cancer. A true fact, and a conclusion that does not follow from it

In August 2026 a claim began circulating that the brush used in a cervical smear test is sterilised in a cancer-causing chemical, and that having the test is therefore dangerous. Full Fact checked it on 17 August, after the posts had been shared hundreds of times across Facebook, Instagram and X.

The claim opens from solid ground, which is exactly what makes it durable. Ethylene oxide is a real gas, it really is used to sterilise swabs, speculums, wound dressings and a great deal else, and overexposure to it really can cause cancer and, in severe inhalation cases, lung damage. Anyone who checks those statements will find them confirmed.

What does not follow is the step from there to the patient. Asked about the cervical screening version, a spokesperson for the Medicines and Healthcare products Regulatory Agency told Full Fact there is no credible evidence that the swabs used in cervical smear tests are associated with cancer, and that negative health effects would involve higher and prolonged exposure. Full Fact's verdict is that overexposure can be dangerous while devices sterilised with the gas are not unsafe.

The thing that is applied and then taken away

The whole argument turns on a distinction most people have never had reason to learn: a processing agent is not an ingredient. Sterilisation is something done to an object and then undone, and the question is not whether the gas was ever present but whether any of it is still there when the packet is opened.

Professor Andrea Sella, professor of inorganic chemistry at University College London, has described the process as exposing a material to ethylene oxide for a few seconds or minutes and then removing the gas entirely before the now sterile item is packaged. There is nothing left of the ethylene oxide once it is packaged, he said, and no conceivable harm from using an item sterilised that way. Dr Alexander Edwards, associate professor in biomedical technology at the University of Reading, has told fact checkers that before any medical device leaves manufacture it is carefully checked to ensure no residual ethylene oxide remains.

This is not an exotic principle, and it is worth noticing where else it turns up. The day before Full Fact published, the Brazilian fact-checking site Boatos rated as a hoax a claim that Oreo filling is made from petroleum-derived compounds. That claim runs on the same error: hexane, a hydrocarbon obtainable from petroleum, genuinely is used to extract vegetable oils, and the posts treat a solvent used in a factory as though it were an item on the ingredient list. Two claims in two days, in two countries, about a biscuit and a medical device, both built on the same confusion.

What the evidence shows

Who checks, against what, and how you can look it up

The strongest answer here is not a reassurance but a document. In May 2021 the MHRA published its response to a Freedom of Information request about ethylene oxide and the swabs used in lateral flow tests, and it lays out the chain.

The manufacturer is responsible for testing how much ethylene oxide is left on the device after sterilisation, against an allowable limit set by the international standard ISO 10993-7. That documentation is reviewed by an independent third-party organisation, an Approved Body in the UK and a Notified Body in Europe, before the manufacturer is permitted to supply the market. The Approved Body then audits the manufacturer's process periodically, and the MHRA in turn audits the Approved Bodies. The permitted residue is not one figure either: it is set according to how long the device is in contact with the person and what kind of contact it is, across limited, prolonged and permanent categories.

The most useful detail is the smallest. According to the same response, a swab may be sterilised with either ethylene oxide or radiation, and which one was used is printed on the packaging: a rectangular box reading STERILE, followed by EO for ethylene oxide or R for radiation. The Approved Body responsible has a unique number displayed beside the CE or UKCA mark, and searching that number returns the body's name and contact details. It is hard to sustain a claim that something is being hidden when it is stamped on the wrapper in a standard symbol.

What the evidence shows

The claim is older than the smear test version

This did not begin with cervical screening. In March 2021, as lateral flow testing was rolled out to schools and colleges in England, a large number of posts warned people not to use the tests because the swabs were sterilised with a cancer-causing chemical. Full Fact checked it then and reached the same conclusion it reached in 2026.

Variants followed and were checked one at a time: that ethylene oxide on a PCR swab would harm a pregnant woman, and that the sterilising chemical would convert into antifreeze inside the body. Each was answered separately.

That history is itself evidence about what the claim is really tracking. An argument that moves without modification from a nose swab, to a pregnancy test, to a cervical brush is not responding to anything about those procedures. It is attached to a manufacturing step they happen to share, and it will attach to the next product that shares it too.

Why people believe

Why this one is more persuasive than most

Most viral health claims fall apart the moment anyone checks the first fact. This one survives that test, because the first fact is true. Ethylene oxide is a carcinogen; it is used on the swab. A person who verifies that much has been rewarded for checking, and has no obvious signal telling them to keep going.

The setting compounds it. Cervical screening is intimate, uncomfortable, and easy to put off, and a warning about a hidden carcinogen offers a far more respectable reason to postpone an appointment than not wanting to attend. A claim that supplies permission is doing something different from a claim that merely supplies information.

There is also the shape of the disclosure. Nobody is handed a leaflet about sterilisation chemistry before a smear test, so a post that explains it feels like someone finally telling you the truth. In fact the information was published by the regulator years ago and abbreviated on the packaging, which is the ordinary way technical facts reach patients: available, standardised, and not announced.

What the answer costs, and what the question is worth

The people sharing this are not, for the most part, hostile to medicine. They have encountered a genuine carcinogen in an unexpected place and want to know whether it is about to be used on them. That is a reasonable question, and it has an answer: the gas is applied, then removed, the residue is capped by an international standard, the manufacturer must test for it, an independent body reviews the testing, and the method is marked on the box.

The cost of the wrong answer is not symmetrical, which is why the distinction is worth insisting on. Cervical screening detects a cancer that is treatable when caught early. A claim that makes the test itself sound like the hazard is not a neutral error, and Full Fact says as much when it notes that false or unevidenced health claims can cause harm when people make decisions based on them.

None of which is a reason to be told what to do by a website, including this one.
